files/folder permissions or ownership are changed frequently

Hi,
We have Windows Server 2008 R2 file server.
For example each departmental folders has:
for SALES folder
- administrators - full control
- sales staff - modify
- company staff - read & execute

for MARKETING
- administrators - full control
- marketing staff - modify
- company staff - read & execute

We are frequently encountering an issue where users are unable to access one of the child/subchild folders/files from the departmental shares.

A quick resolve: I normally force the permissions from parent folder i.e. Replace all child object permission with inheritable permission from this object

Checking the affected folder/files what I can see is the ownership of the folder/file has been changed to some user (user1) and the security permissions has been modified where user1 is listed on the Security tab. User1 claimed that he/she hasnt changed the permission but this sort of issue is happening quite regularly and sometimes its user2 or user3 or user4 etc; And everyone is saying that they have never changed any permissions.

I dont know how this is happening, Is there any way these sort of issue can happen ?
