Windows 2003 Server migration best practices\recommendations

To preface,  I am not new to IT, but new to the role of Network Administration and have never had the pleasure of upgrading a DC with next to no help.  Here is what I would like to accomplish.

I have a Windows 2003 Server DC running DNS, DHCP, AD and it is a File server.  It handles less than 50 AD users.  I would like to migrate to Windows 2012 Server.  The new server will be a physical standalone box (3 or more drives running RAID 5) for now.  VM can be debated some other day.

Is there any recommendations in making this migration that others in this community have experienced?  Such as guidelines for the migration of AD, GPO, File Server etc... I know there are a lot of videos on utube,  but I was looking for some real world scenarios.
