Disk defragmentation not completed in windows server 2012

I have a server with windows server 2012. It has 4 disks with 4 partitions in each. In that 16 drives, 4 are configured with volume shadow copy. Everyday the data in those disks with shadow copy is enabled will be modified. So I need to defrag the drives once in a month or twice.

But my problem is defragmentation is not completing in one of those disks. I tried with several type of defragmentation like through command, using external defragmenter utilities. But still problem not solved. Same error was there in another server also. It was windows server 2003 R2. In that server after completing defragmentation it was giving error report. The report was like this. Cannot defrag "system volume information" folder. So I disabled the shadow copy for that drive. Automatically the fragmentation % was reduced. Same case happening here also.

What may be the problem? How to troubleshoot this? There is no much size on the disk. 120 GB free out of 250 GB drive.
